The hours are counted as the monthly payout run counts them: lessons starting in the month, paid or transferred, weighted by the lesson size. 'actual' re-bands them with the limits in force in that month, 'settled' reports the band the lessons were really settled at, 'simulated' bands them with the proposed limits (omitted when none are supplied).
